About Us

The staff at Avamere Rehabilitation of Junction City provides Skilled and Long-term care to seniors and persons who require post hospital rehabilitation services. These services include 24/7 on-site nursing care, Physical, Occupational, and Speech therapy. Our staff has the knowledge, skills and abilities to provide complex wound care (including wound Vac therapy), Pain management, Trach care, tube feeding services, stroke care, IV therapy and the special needs of Bariatric residents. A person-centered plan of care is developed for each resident designed to meet their needs and expectations. Long-term Care in Junction City has never been this comprehensive.

The staff at Avamere Rehabilitation of Junction City is dedicated to enhancing the life of every person we serve and meeting our residents wants, needs, and expectations. We also provide services to meet the psycho-social needs of our residents through an extensive activities program that is enhanced by our community-based volunteer program and on-site beauty shop services.78620132 (Small)

We contract with a psychologist nurse practitioner to assist in the care of behavioral issues and medication management. In addition to our focus on the daily nursing care of our residents, Junction City Rehab also provides comprehensive planning assistance in returning our residents to their homes; including home assessments, arranging for Home Health or Home Care when needed, providing outpatient therapy services for those who may require additional assistance but can live at home.

Avamere Rehabilitation of Junction City also provides respite services for short stays up to 14 days to provide in-home caregivers the opportunity for a much deserved rest. The Staff is also trained in and works closely with our Hospice agency to provide care and support to the resident and their families.

Avamere Rehabilitation of Junction City has provided skilled and long-term nursing services to our community for over 25 years. The community support has grown over the years, providing us with a large number of volunteers and businesses that donate time, energy, and activities for our residents.

One of our largest volunteer contributors is Guaranty Chevrolet who provides Christmas gifts annually to all of our residents. We collaborate with Peace Health and McKenzie Willamette hospitals to provide a seamless transition to our facility. The local VFW and Moose Clubs provide activities and support on special occasions including Veteran’s Day and Flag Day.
